100% Remote in the 28 states - North Carolina, Alabama, Arizona, Arkansas, Colorado, Florida, Georgia, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maryland, Michigan, Mississippi, Missouri, Ohio, Oklahoma, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, Wisconsin, and Wyoming will be allowed to maintain their current residence and work remotely.

Job Description Summary
The Peer Support Specialist utilizes their training and lived experience to effectively coordinate care
for members diagnosed with substance use disorders and/or serious mental illnesses, promoting improved member experience, quality of care outcomes, and cost efficiency.
Serves as a resource to the interdisciplinary care team, advocating for member needs and ensuring linkage to services and resources that best match recovery progression.

Job Description
Serve as an active member of a multidisciplinary team, helping identify and facilitate
appropriate behavioral health care services.
Conduct outreach to members identified as high-risk, high-utilization, or with complex
behavioral health needs.
Encourage early and proactive interventions that support recovery and reduce preventable costs.
Complete intake assessments and assist with development of person-centered care plans to identify member support needs.
Provide ongoing outreach, encouragement, and recovery-oriented support as recommended
by the Behavioral Health Case Manager, including follow-up on goals, plans, and progress.
Teach and reinforce empowerment skills that promote self-advocacy, recovery, and
resilience.
Assist members in identifying and accessing community resources, explaining processes
and supporting follow-through with next steps.
Encourage development of healthy relationships by supporting involvement in social and
recreational activities aligned with interests and recovery goals.
Make referrals to appropriate services and document all actions accurately in the electronic medical record.
Support members in everyday decision-making and the resolution of minor problems or
barriers.
Participate actively in care team meetings and collaborate continuously with team members.
Manage task lists and daily outreach goals to support timely, organized member
engagement.
Appropriately share personal lived experience to instill hope, model recovery, and support
member engagement.

What You'll Bring:
High school diploma or GED
Must be a Certified Peer Support Specialist in NC
0-2 years of experience in related field
