Abound Health is seeking an Intake Coordinator to join our team supporting individuals and families entering Innovations Waiver and State-Funded Services. In this role, you will report to the Intake Manager and serve as the first point of contact for clients and referral sources, ensuring a smooth, welcoming, and efficient intake experience while upholding Abound Health's mission and values.
Key Responsibilities:
Serve as the primary point of contact for referrals and intake inquiries within assigned counties
Collect, review, and coordinate intake, referral, and admission documentation
Communicate regularly with families, referral sources, and internal teams to expedite intake timelines
Coordinate with Program Directors and clinical teams to ensure smooth client transitions
Identify staffing needs and communicate with Staffing and Onboarding teams
Participate in intake-related meetings and advocate for clients and families
Ensure accurate documentation is uploaded and maintained in required systems
Qualifications:
Credentialed as a Qualified Professional (QP) under one of the following:
Master's degree in a related human services field with 1 year of IDD experience
Bachelor's degree in a related human services field with 2 years of IDD experience
Bachelor's degree in an unrelated field with 4 years of IDD experience
Strong attention to detail and organizational skills
Excellent written and verbal communication skills
Ability to manage multiple priorities in a fast-paced environment
Collaborative, professional, and client-focused approach

This is a full-time, salaried, exempt position. Compensation is competitive and based on experience, education, and job-related skills. Abound Health offers a comprehensive benefits package, including health, dental, and wellness benefits beginning on the 60th day of employment, as well as a 401(k) plan available after 12 months of employment.
