Mental Health Kokua (MHK) is seeking a Full-Time Case Manager in Kona to support adults with mental illness living in the community. In this role, you partner with individuals to coordinate services, build life skills, and support recovery goals—helping people maintain housing stability and meaningful connections while navigating community-based systems of care. Case Manager Responsibilities Care Coordination: Assess client needs, develop service plans, and coordinate behavioral health, medical, and community-based services. Client Support & Advocacy: Support individuals in building skills, accessing resources, and navigating systems of care. Housing Stability: Assist clients with maintaining housing, addressing barriers, and developing independent living skills. Documentation & Compliance: Complete timely, accurate documentation including assessments, service plans, and progress notes. Community Collaboration: Coordinate with providers, landlords, family members, and community partners to support client goals. Crisis Support: Respond to client crises as needed and follow MHK policies and procedures.
