LOCATION: KONA COMMUNITY HOSPITAL | Social Work Services

POSITION STATUS: 0.30 | Temporary

SALARY RATE: $28.6200 (Hourly)

SHORTAGE DIFFERENTIAL: $2.02/an hour

JOB DUTIES:

A) Functions as a member of the Interdisciplinary Team, working closely with Case Management, Utilization review and the Unit Managers, supporting all of the patient care areas of the hospital.

B) Takes part in the care planning process by attending Care Progression Rounds and participating in other venues that promote appropriate discharge planning and continuity of patient care.

C) Offers supportive services to indigent, abused or displaced patients utilizing appropriate standards of Social Work.

D) Works closely with all patients, their families, and other significant persons in order to enable them to deal with the impact of illness on individual and family functioning. Maintains awareness of the fact that the hospital may not be the appropriate place for a patient, thereby facilitating in a timely manner a transfer or discharge to another facility or level of care more appropriate to the patient's needs in a timely manner. Proactively recommends options for care, including internal and external community/government resources, in developing short-term and long-term care plans.

E) Connects patients with resources, agencies or services identified as needed for the patient or family during the Social Work assessment, care progression rounds, etc. including but not limited to home health, hospice, bereavement counseling, mental health, substance abuse services, etc.

F) Completes assessments and documents findings and ongoing issues that include the following:

1. A social history and assessment of current social and emotional needs.
2. A current social work plan to meet identified needs.
3. Regular progress notes indicating the patient's status.
4. Appropriate discharge plans.
5. Evidence of regular review of social work and discharge plans in conjunction with the overall plan of care, including prior level of functioning, barriers to discharge, and the resolution or plan to resolve these issues.

G. Assists with placement, transportation, acquiring durable medical equipment, and referrals for outpatient services.

H. Assists patients to complete an Advance Directive, if desired.

I. Works with patients who have legal encumbrances that relate to their stay in the hospital. Assists patients and families in advocating to receive appropriate services within the community.

J. Works closely with the legal system regarding mental health civil commitments. Collaborates with other disciplines in assessing, planning and providing services for patients utilizing bio psychosocial information.

K. Provides crisis counseling and responds to the need for support and guidance when someone has lost a loved one.

THE MINIMUM QUALIFICATION REQUIREMENTS ARE: Applicants must meet all of the following requirements. Please note that unless specifically indicated; the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week.

Education Requirement: Graduation with a bachelor's degree or master's degree in social work from a program of study accredited by the Council on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges, or a comparable regional accreditation body.

Specialized Experience: One (1) year of progressively responsible professional work experience which involved helping individuals and their families find satisfactory ways of identifying their problems, coping with their conditions, and functioning effectively within their environments. Depending on the employment setting and the kinds of clients or patients served, job duties and responsibilities may vary, although typically the work will include the identification and evaluation of the client's or patient's problems and needs; the development of a service or treatment plan, sometimes in tandem with other professionals working in an interdisciplinary team; the initiation and implementation of the service plan, either directly or through the authorization of provider/vendor services; monitoring of services being provided; and evaluation/assessment of the client's or patient's progress, with amendments to the service/treatment plan made as appropriate.

Knowledge and Abilities Required: In addition to the knowledge and abilities required to perform the work of this series, persons with social work degree, by virtue of their education and training also bring with them knowledge of principles, procedures, techniques, trends, research, values, ethical standards and practices of professional social work; theories, methods and techniques of psychosocial assessment and diagnostic impressions in order to evaluate and provide treatment for the social service needs of individuals, couples, families and groups to prevent psychosocial dysfunction, disability or impairment including emotional, mental and behavioral disorders; social casework principles, practices, methods and techniques, including group work, community organization and planning; social work research methods and techniques; and the ability to apply professional social work knowledge, principles, ethics and skills in direct practice with individuals and groups, and in supervising other social workers and healthcare services specialists; understand and demonstrate skill in the application of social work principles, methods and practices; and effectively communicate the principles of social work practice or a specific discipline within the practice of social work.

Driver's License: For certain positions, applicants may be required to have a driver's license.
