Required Qualifications
Phlebotomist, Laboratory Experience: 1 year Certifications: BLS-AHA(Copy needed) SSN Required DOB Required Work History: Should not be more than 90 days of gap in last 1 year Please READ BEFORE SUBMITTING AND CONFIRM TRAVELER IS CLOSED ON PAY Please close your travelers on pay prior to submitting, rate is $60-4% and will need a car rental since they will be covering multiple sites Housing is also expensive Asap 26 weeks Must be available for five 8-hour shifts from Sunday - Saturday to cover any shift, including weekends/holidays. Must be flexible with operational needs. $60-4% Will be required to cover the following locations on the Big Island: &bull
tKona Community Hospital (79-1019 Haukapila St, Kealakekua, HI 96750) &bull
tNorth Hawaii Community Hospital (67-1123 Mamalahoa Hwy., Suite 116, Kamuela, HI 96743) &bull
tPatient Service Center (PSC) Kona (75-170 Hualalai Rd, Suite C110, Kailua-Kona, HI 96740) No housing, travel, car or transportation expenses covered. Must be paid for by agency's all-inclusive rate. Training will occur on the Big Island.
