About You

You are a compassionate and skilled Nurse Practitioner or Physician Assistant with an active California state license and board certification. New graduates are welcome, provided you have relevant clinical experience as a Registered Nurse (RN). You are comfortable conducting virtual assessments and have a passion for working with elderly patients. You possess excellent administrative, organizational, and communication skills, particularly with older adults. You are detail-oriented, dependable, and able to work independently, ensuring the highest standards of care are maintained.

What You'll Do

As an Advanced Practice Provider, you will:

• Conduct virtual assessments, including past medical history, review of symptoms, physical examinations, medication reviews, and cognitive/depression screenings.
• Provide patient education on health monitoring and community resources, promoting independent and informed patient choices.
• Evaluate vitals, BMI, and perform virtual physical exams (excluding blood draws), formulating a list of current and past medical conditions.
• Communicate assessment findings to the primary care physician to inform potential gaps in care.
• Educate members on disease processes, medication, and compliance.
• Comply with all HIPAA regulations and maintain the security of protected health information (PHI).
• Work a schedule from November 4th to December 22nd, conducting virtual visits 5 days a week, with 4-5 patients per day, starting at 8 am. Typical shifts include time for documentation and patient follow-up.
